Tailored care for pregnancy
Congratulations, you are pregnant. It goes without saying that you want the best care for you and your baby. It is therefore good to know that the nine regional midwifery practices, gynaecologists, midwives, paediatricians and neonatologists from the Reinier de Graaf hospital have developed an integrated care model that means they are working together more intensively. It is almost always possible to have your baby at home or in the hospital This care model is called the Reinova Geboortegroep and helps to improve the quality and safety of care in the Delft area.
Tailored care
You will have your first appointment with your midwife in a midwifery practice. However, if you have a clotting deficiency or you have had an ectopic pregnancy or multiple miscarriages in the past, then your first appointment will (possibly) be at the Reinier de graaf hospital.
As a pregnant woman in Delft it is also nice to know that the kraamzorg bureaus, JGZ (child health team) and the gynaecologists, midwives, paediatricians and neonatologists from the Reinier de Graaf hospital are working closely together via the VSV Reinier (Verloskundig Samenwerkings Verband). There is regular contact between all of these specialists in order to optimise the care for you. We as midwives discuss your pregnancy (with consent) in a meeting with the gynaecologists and the midwives from the hospital (Multidisciplinary meeting). If necessary, a paediatrician or neonatologist, kraamzorg and/or the baby clinic from the JGZ will be consulted. During this meeting a risk analysis will be made. Based on the we will decide which care path suits your needs best, tailored care. This care path contains the information about what care you will receive and where. With the midwife where possible, with the gynaecologist if necessary.

In order to provide the best possible care during our pregnancy, birth and postpartum, it may be necessary to share your medical details with other care givers in the Reinier Geboortegroep. We as you therefore to sign a consent form to allow us to do this.
Family centred care
Should you choose not to have your baby at home or due to circumstances it becomes necessary to give birth in the hospital, then you can make use of the Midwifery/Postnatal department of the Mother and Baby Centre in the Reinier de Graaf hospital. You will stay here together with your partner in a birthing suite and can have your baby in the way that you choose. All care given in the Mother and Baby Centre centres around the family. After the birth you will stay in the birthing suite or postnatal suite with your baby and partner. If your baby needs extra care then you will move together to the Neonatal department of the Mother and Baby Centre. Here we also offer family centred care. Kraamzorg is also a part of this care. Whether you stay on the Midwifery/Postnatal unit or the Neonatology unit, our care professionals will coach you and your partner in the care for your baby so that you can go home together with confidence.
